I'm trying to fit a new EMG P pickup to a Warwick Adam Clayton bass with no success. I followed the accompanying diagram but still no luck? Another issue is the EMG output jack is a lot smaller than the stock Warwick one it comes with. Any ideas?

Look carefully at the official EMG diagram, you'll see what you're doing wrong!

The pickup is entering the volume pot below, not above and you mismatched the position of the connectors on the tone pot!?!

So the inputs and outputs are totally wrong.

Double check everything before going any further.

Indeed, the pickup wire is going to the volume, but at the wrong place...

I think you don't understand a wiring diagram as you didn't follow it at all.

The position, place and direction of each connector is mandatory.

Jack sockets for active basses need 3 connections - tip, ring and sleeve. This means that whenever a Jack lead is plugged in it forms a connection between the ring and sleeve which completes the power supply circuit - typically the ground connection, leaving the tip to carry the signal. This has the advantage of not drawing power from the battery whenever you unplug your lead. If the original socket only has 2 lugs then you’re going to need an awful lot of batteries…

Maybe difficult to tell from the photo but this is the passive Warwick jack, it looks to have 3 connections. On that basis, do you think it would work for and active EMG pickup?
