It's simply maxed in, and an easy trick is to simply put some flat washers under the bullet with some graphite grease to get it work as it should.

 

It's something that happens very often with old Fender necks.

Try the securing washer from an old bicycle tyre valve. The old-fashioned sort of bike valve washer has exactly the correct dimensions. It is quite thick, but I would judge the truss rod would be restored to a flush position by doing so.
