PCB is from the guitar pcb website.It was listed as "Guitar and bass driver" and it is a Catalinbread SFT derivative with "Bass", "Treble","Volume", "Gain" and the extra "Boost" potentiometers. Also has a switchable capacitor circuit to select for Bass or Guitar. Untested, but never powered so should be easy with someone with pedal building/electronics experience to finish. Mainly just needs the pots wiring and all the wiring and PCB th0roughly checking over.
