This is an original Music Man preamp, 2EQ, from 2010. 
 

Comes with the original input jack too. 
 

I have left two wires attached to the preamp in case you do not want to solder the pickup wires direct to the circuit board. 
 

I have an original banana plate that I may consider selling too should you need it.
