Just Got My Focusrite Pro 40 back from repair apparently the FYI chip as failed plugged it in and the same thing as happened again or so I believe because there are the same symptoms.
Do you think it could be my PC because it was fine when I was using the onboard FW port but it stopped working when I used a PCIE card.
Can anyone shed some light on this please?

I suspect it's the Firewire PHy. This is the chip that connects the Firewire port to the internal processing. Some designs are vulnerable to being 'hot plugged' i.e. plugged in with the power on. There's some comments about it here.

Generally, if it can be avoided, try not to plug firewire devices in with the power on.
