Is this going to require a transformer to convert the 220V to 120V for the US? -

Which Muff Pedal are you using: Guitar Muff Bass Muff Russian Muff Sovtek Muff (not sure if it's the same as a Russian Muff) Those are 4 Muff Pedals that I know of, unless there are some that I never knew existed
