LA Audio high quality active DI box, very solid construction; comes with an instruction leaflet - which is worth reading since the box is deceptively versatile!

This works off an 18v rail so can cope with high or low level signals. Power via phantom from your mixer or 2 x 9v batteries

Usefully it has a +20db switch to increase the level of low output devices and with 1M ohm input impedance is compatible with piezo pickups 

 

The Input will work with Instrument or Speaker levels ('Pad' switch is for speaker level), isolated XLR output for your mixer and a 'link' output to take the signal on to an amp or cab 

A separate unbalanced output is available as well as the balanced XLR
